Sleep Log Instructions
- Be sure to enter your name on your sleep diary
- Circle the day of the week and enter the date in the page provided for each 24 hour period. Complete one week per diary page.
- For example, if you begin on a Wednesday, your first recording will be Wednesday and your last diary recording will be the following Tuesday.
- With a solid box, block each 1/2 period you slept without waking for more that 2 minutes. If you were awake for more than 2 minutes during any 1/2 period of rest, do not block in the box for that 1/2 hour.
- To figure out the total time asleep for the day, add up all the fully blocked 1/2 hour squares and then record that number at the far left column under "total hours asleep."
- Using the behavior codes at the bottom of the diary page, record the code (s) that best describes your behavior during each hour of the day.
- Note and describe any behavior that you feel was unusually significant, even if it lasted less than one hour or even less than ½ hour.
- If a behavior lasts for more than 1 hour you can enter the behavior code in the box above the hour and then indicate additional hours that the behavior lasted by drawing a line in the behavior code box above the hour (s).
- If you are using the behavior code "E" which represents exercise, the "E" means any activity requiring strenuous exertion (tennis, dancing, labor, as well as obvious exercise like jogging, aerobics, weight lifting, etc). The behavior code "U" indicates upset and includes markedly unpleasant events.
- Use a bold diagonal line in each 1/2 period when sleepy.
